We are pleased to inform you that the Umrakh Primary Health Centre (PHC) is organising a Td (Tetanus and Diphtheria) Vaccination Programme in Freedom Valley School for eligible students on 18 July 2026.
Who is Eligible?
Students who have completed:
10 years of age & 15 years of age only
Vaccination Schedule: Date - 18 July 2026, Saturday. Morning 10 am
Venue: School Library
Why is the Td Vaccine Important?
The Td vaccine protects children against two serious diseases:
Tetanus: A bacterial infection that affects the nervous system and can be life-threatening.
Diphtheria: A contagious disease that can cause severe breathing difficulties, heart complications, and other serious health problems.
Receiving the Td vaccine at the recommended age strengthens your child's immunity and provides long-term protection against these diseases.
Parent Consent
We kindly request all parents of eligible students to complete this Google Form and submit the online consent form by 16 July 2026. Submitting the Google Form will be considered as your consent for your child to receive the Td vaccination during the school vaccination programme.
